Abstract
Introduction: Abdominal trauma is a global public health concern, particularly in developing nations like Iran, where rapid urbanization and increased motorization have elevated trauma burdens. Understanding the epidemiology and clinical outcomes of these injuries is vital for refining management and preventive strategies. This study aimed to investigate the characteristics and clinical outcomes of patients with abdominal trauma presenting to the trauma center of Imam Hossein Hospital, Shahroud.
Methods: This descriptive-analytical, cross-sectional study included patients with abdominal trauma admitted to Imam Hossein Hospital (2019–2021). Data regarding demographics, injury characteristics, management, and clinical outcomes were extracted via a structured checklist. Statistical analysis was performed using SPSS (version 18), employing independent t-tests, ANOVA, and Chi-square tests. A P-value < 0.05 was considered statistically significant.
Results: Of 196 patients (69.4% male), 92.9% experienced blunt trauma, primarily via traffic accidents (66.3%). Penetrating injuries (7.1%) occurred exclusively in males, linked to interpersonal violence. Emergency surgery was required in 36.7% of cases, with 33.7% requiring ICU admission and a 6.1% mortality rate. The mean hospital stay was 8.7 days. Patients with blunt trauma were significantly older than those with penetrating injuries (P=0.001). Penetrating trauma was associated with a higher need for emergency surgery (P=0.001) and ICU admission (P=0.007). Clinical outcomes significantly correlated with marital status, injury mechanism, and surgical intervention (all P<0.01), whereas sex, underlying diseases, and substance use showed no significant association.
Conclusion: Abdominal trauma in this setting primarily affects economically active males, driven by traffic accidents and violence. These findings emphasize the necessity for targeted preventive strategies, including stricter traffic safety enforcement, public education, and community-based violence intervention programs to reduce the morbidity associated with abdominal trauma..
